Request a Condition Plan

If a family member has been diagnosed with a chronic condition, you can request enrollment in a DiaspoCare condition plan. A Community Health Worker in their area will follow up within 5–10 business days to verify eligibility and begin enrollment.

Covered Conditions

  • Hypertension (high blood pressure)
  • Diabetes (Type 1 and Type 2)
  • Kidney disease (chronic kidney disease)
  • Asthma
  • Dialysis

How It Works

  1. Submit your enrollment request with your family member's condition and location.
  2. A CHW in their region reviews the request and contacts your family within 5–10 business days.
  3. Once verified, the condition plan is activated on the same wallet and ledger you use for screenings.
