Beyond Screenings — Ongoing Chronic Care Management

Screening finds problems. Chronic care solves them. DiaspoCare covers ongoing care for hypertension, diabetes, kidney disease, asthma, and dialysis. Once a family member is enrolled in a condition plan through our Community Health Worker network, their medication, care visits, and adherence check-ins are funded from the same wallet and shown in the same ledger. Every plan tier includes access to chronic care management. The tier determines how many family members can be enrolled at once.

A $10 screening splits as: $1.74 consumables, $1.52 facility and staff, $6.74 net split. Shown in full before every booking.
